Appointment Information
Please bring all of the following items with you to your appointment:
- A copy of any prior authorization or referral, if required by your insurance.
- Insurance card(s).
- Drivers license for the patient or responsible party.
- Any X-rays, MRIs, or other test results related to this injury.
- Workers’ Compensation information (if applicable).
- Employer’s name, address and telephone number.
- Insurance Carrier name, address and telephone number.
- All Injury Information.
Time and preparation go into your visit! If you are unable to keep your appointment, please give us 48 hours notice.
To make or change an appointment call (928) 537-8777. Our office hours are 8:00am to 5:00pm Monday through Thursday, and 8:00am to 12:00pm on Friday, with the exception of major holidays. We close for lunch and these times will vary.
If you have an urgent need outside of our regular office hours, please call (928) 537-8777. An orthopedic doctor is on-call 24 hours a day.
